Job Description:

CentraCare is looking for a provider to work in our family practice setting at our Saint Cloud Medical Group Northwest campus. This position will have a focus on wellness, chronic disease management...etc. The candidate will work 64-80 hours every pay period. We are flexible to what the selected candidate wants to work within this structure. Physician Assistant The Physician Assistant provides primary health care and performs selective medical services under the direction of clinic physicians to ensure effective and efficient delivery of care to patients of St. Cloud Medical Group. The Physician Assistant follows established protocols and collaborates with physicians, however, does have day-to-day independence in health care decisions. Establishes a philosophy of patient and family centered care in all patient care practices. Maintains professional competencies by attending educational programs, participates in professional organizations, subscribing to the reading professional journals or other appropriate activities. EDUCATIONSuccessfully completed a formal physician assistant program approved by a national accrediting agency. EXPERIENCEPrevious experience as a Physician Assistant is preferred. SPECIAL REQUIREMENTSMust have a current Physician Assistant (PA) license in Minnesota. Certified by a national certification organization recognized by the State of Minnesota.Successful completion of medical staff office credentialing must be completed prior to working without a preceptor. Successful completion of third party payer credentialing must be completed prior to billing for services. Current American Heart Association (AHA) BLS Healthcare Provider card required per established policy.Registered DEA number. Nurse Practitioner The Nurse Practitioner provides primary health care and performs selective medical services under the direction of clinic physicians to ensure effective and efficient delivery of care to patients of St. Cloud Medical Group. The Nurse Practitioner follows established protocols and collaborates with physicians, however, does have day-to-day independence in health care decisions. EDUCATIONSuccessfully completed a formal nurse practitioner program approved by a national accrediting agency. EXPERIENCEPrevious experience as a nurse practitioner is preferred. SPECIAL REQUIREMENTSCurrent licensure as a registered nurse in the State of Minnesota required.Current certification by a national certification organization recognized by the Minnesota State Board of Nursing.Current licensure as an APRN (Advanced Practice Registered Nurse) in the State of Minnesota required.Successful completion of medical staff office credentialing must be completed prior to working without a preceptor.Successful completion of third party payer credentialing must be completed prior to billing for services.Current American Heart Association (AHA) BLS Healthcare Provider card required per established policy.Registered DEA number.
